Paperwork and physical information for those interested in participating in Winter Sports at LVMS is due by Friday, October 23rd. When accessing the forms through the Genesis Parent Portal, once accessing the forms, please be sure to select a sport your student wishes to tryout for / participate in
The Single Page Medical Form is what is REQUIRED to be completed by a physician and uploaded / submitted
All other sections of the Sports Forms MUST be completed in Genesis (no additional download / uploaded forms are required) - Health History Update, Athletics Sign-Off Sheets (Concussion, Opioid Fact Sheet, Eye Injuries Fact Sheet, Sudden Cardiac Pamphlet, Eligibility Acknowledgment, and Athlete Baseline Concussion)
Once the Single Page Medical From is completed and uploaded, and the additional forms are completed / acknowledged please select "Update Answers" and submit the information for review by our nurse and district physician. All fields marked with a * are required to be completed prior to submitting.

ALL FORMS MUST BE SUBMITTED THROUGH THE GENESIS PARENT PORTAL! PAPER COPIES OF FORMS AND LATE FORMS CANNOT BE ACCEPTED!
