Any student that is interested in participating in Fall 2026 Sports at LVMS, must submit all required paperwork and physical information by Friday, August 14th, 2026. Paperwork cannot not be accepted after this date.
FOR THE 2026-27 SCHOOL YEAR, ALL FORMS WILL BE FOUND ON THE GENESIS PARENT PORTAL
ALL FORMS MUST BE SUBMITTED THROUGH THE GENESIS PARENT PORTAL.
PAPER COPIES OF FORMS AND LATE FORMS CANNOT BE ACCEPTED!
