Please see the included memo and permission slip from LVMS Nurse Mrs. Pinto regarding the annual Scoliosis Screening that is scheduled to take place in the fourth week of September. Please CLICK HERE to access the form and permission slip, which includes additional information. Please complete the form and have your student return it to their homeroom teacher or email your response to Mrs. Pinto directly at kpinto@wtschools.org. It is requested that all forms be returned to LVMS by Monday, September 18th. Please be aware, if a response is not received, Scoliosis Screening will not take place and the student record will be updated as declining this screening.
